§ 401.200 — Application For Registration

An application for registration as a U.S. Registered Pilot must be made on Form CG-4509, which must be submitted via email to: GreatLakesPilotage@uscg.mil; or by physical mail to: Great Lakes Pilotage Office, 2703 Martin Luther King Jr. Ave SE, Stop 7509, Washington, DC 20593-7509. This form may be obtained from the Director.
